As part of the BIFS study, you agreed to complete an annual questionnaire. This takes about 10 to 15 minutes and includes questions about the following:

• Any lifestyle updates (eg, smoking and drinking habits)
• Medical diagnoses (eg, connective tissue, neurological, cancer, mental health)
  • Contact information for physicians is also requested
• Reproductive health, including whether you are pregnant or breastfeeding
• Medical information for any children born since you started participating in the BIFS study
• Medications you take
• Implant complications, re-operations, or removals
• Any symptoms you are experiencing

If you are unsure about certain medical information, we recommend you reach out to your physician(s). If you are not able to remember exact dates, as requested on certain questions, please provide approximations with the month and year.
